Program Facilitator (SBIRT)

Qualifications: The required qualifications include the following:

  • Minimum of Master’s Degree in Social Work, Counseling or other related human service field
  • Automobile and valid driver’s license or access to transportation
  • U.S. Citizenship or work permit
  • Knowledge of the county and its culture
  • Computer skills – proficient with Microsoft Office, email, etc.
  • Strong interpersonal communication skills
  • Able to work independently and efficiently

General Statement of Duties: The Program Facilitator will work with the Morgan County Partnership’s Screening, Brief Intervention and Referral to Treatment (SBIRT) Program and Morgan County Schools to help identify and provide help to students with a substance abuse program and/or mental health issue. This will include, but is not limited to the following:

  • Develop and facilitate strategies to implement the SBIRT program in Morgan County Schools
  • Screen students, using CRAFFT, aged 12-18 throughout the school year and during Health Screen Days twice a year
  • Provide brief interventions to students, as needed
  • Refer high risk students to outside treatment providers and/or to the PASS mentor program
  • Work with Morgan County School’s Social Worker to ensure a seamless tranistion between each stage of the SBIRT process
  • Create and provide informational material during various wellness events and distribute to local businesses
  • Become familiar with community resources by attending organizational meetings and one-on-one meetings as needed
  • Track results of screenings, interventions and treatments and complete all necessary reporting procedures
  • Participate in collaborative efforts with schools and community based prevention programs

Supervision: Reports to the Morgan County Partnership Director for administrative support and the Morgan County School Social Worker for clinical support

Required Skills:

  • Background knowledge and experience in prevention methodology and SBIRT Model
  • Written, oral and interpersonal communication skills
  • Liaison between schools, community resources and the Partnership
  • Advocacy
  • Public speaking
  • Computer skills in Microsoft Word, Excel, Power Point, as well as, internet and social media


  • Demonstrate positive interpersonal communication and effective listening skills
  • Function as a member of an interdisciplinary team
  • Work independently with minimal supervision
  • Establish and maintain effective working relationships with colleagues, families, school system, civic organizations, student groups and community agencies
  • Demonstrate adherence to strict confidentiality and ethical standards
  • Exercise sound judgment
  • Manage multiple work priorities
  • Serve as a professional role model
